Dr. Azar serves as the Chief Quality and Patient Safety Officer at IU Health Adult Academic Health Center and is a founding member of the IU Center for Healthcare Innovation and Implementation Science since its inception in 2012. He is also an Associate Professor of Clinical Medicine at the Division of Hematology and Oncology at a School of Medicine. He served as the Medical Director of Quality Improvement and Patient Safety and the Medical Director for the inpatient Hematology/Oncology Unit at the IUH Simon Cancer Center from September 2010 until June 2014 and then served as the Medical Director of Quality and Patient Safety for the IUH Adult Academic Health Center until 2018 when he accepted his current position.
Dr. Azar is passionate about transforming healthcare into a highly reliable learning system that can accomplish the triple aim of “better health, better care at lower cost”. This is why he co-founded the IU Center for Healthcare Innovation and Implementation Science (IU-CHIIS) in 2012 with the primary purpose of discovering and rapidly implementing innovative healthcare solutions in the real world in order to transform the healthcare system. Dr. Azar and the CHIIS co-founders have developed the novel change methodology of Agile Implementation and Innovation which they published in a book in 2019. He is also a founding member of the Certificate of Innovation and Implementation Science which aims to create a network of healthcare change agents by providing training and mentorship in Agile Implementation and Innovation.
